5 MAJOR AREAS OF FOCUS:
•PROJECT MANAGEMENT
•DEVELOPMENT AND IMPLEMENTATION OF PATIENT CARE TOOLS
•EVALUATION AND UTILIZATION OF DATA
•OPERATIONAL ACTIVITIES
•PROFESSIONAL EDUCATION AND DEVELOPMENT

PROJECT MANAGEMENT
1.Coordinates and defines project scope, goals and deliverables with internal/external stakeholders
2.Collaborates with management to develop, monitor and report on department deliverables: (a) initiates and prioritizes work assignments in an independent manner; (b) submits reports as required within specified time frames.

DEVELOPMENT AND IMPLEMENTATION OF PATIENT CARE TOOLS
1.Assesses and analyzes clinical processes to develop and enhance patient care tools (e.g. Algorithms, Order Sets and Plans of Care)
2.Identifies and evaluates issues related to current standards and processes as it relates to patient care tools
3.Knowledge base and experience in computer applications such as Visio, Adobe Professional and Microsoft Office.

EVALUATION AND UTILIZATION OF DATA
1.Coordinates efforts for evaluation and measurement of patient care tools that align with institutional and national quality standards and/or measures for patient care
2.Develops and conducts audits to ensure compliance with standards for all patient care tools.
PROFESSIONAL EDUCATION AND DEVELOPMENT
1.Directs education and training activities for CE staff
2.Participates with supervisory of staff in evaluating employee compliance with current policies, procedures and/or standard operating procedures
3.Networks and participates in local, regional, or national venues to foster knowledge sharing of patient care tools.

OPERATIONAL ACTIVITIES
1.Coordinates adherence with institutional and industry best practice standards for patient care tools through planning, real-time monitoring, and practicing a well established quality management process
2.Participates in the development, implementation and maintenance of policies, procedures and/or standard operating procedures as required
3.Other duties as assigned.
Job Description-Requirements EDUCATION:
Required: Master's degree from an accredited school of Professional Nursing or a Physician Assistant training program or a Pharm.D. degree from an accredited college of pharmacy.

EXPERIENCE:
Required: Four years of clinical experience to include two years of program or project management experience -OR- Pharmacy Practice Residency (PGY1) and Residency in the area of specialization (PGY2).

LICENSURE/CERTIFICATION:
Current license and/or certification as defined by the TSBPAE or TSBNE as required for clinical licensure as an Advanced Practice Nurse or Physician Assistant or current license to practice pharmacy by the Texas State Board of Pharmacy
